Police on Wednesday were investigating an accident in which a Chicago man was crushed to death by the forklift he was driving. Police said Ernesto Rodriquez, 24, of 2420 W. Arthur Ave. in Chicago, was operating a forklift around 6 p.m. Tuesday at Raja Foods, 8110 St. Louis Ave. in Skokie, when the vehicle skidded out of control. Police said Rodriquez jumped from the forklift, which then fell on top of him.
